* JOB DESCRIPTION:

** The Speech Therapist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.
*** EDUCATION
* *** Possesses a degree, which includes an internship from an approved Master’s program in Speech Language Pathology, or as required by applicable law and/or regulation.
*** EXPERIENCE
* *** 2 years home health care experience preferred.
* Minimum of one-year clinical experience in Speech-Language Pathology required.
* Meets the education and experience requirements for a Certificate of Clinical Competence in Speech-Language Pathology, granted by the American Speech-Language – Hearing Association (ASHA).
*** LICENSURE/CREDENTIALS
**** Current Speech Language Therapy License required.
* Current BLS certification required.
* Valid Washington State driver’s license required.
* Proof of insurance on automobile used for work required.
* Driving record in compliance with OMC standards currently in effect or as may be amended, required.  See HR Policy Driving for OMC Business.
*** OTHER
* *** OMHH issued work cell phone provided.
* Must have computer internet connectivity (OMHH work laptop provided).
* Must have reliable automobile transportation to travel to patient residences.
