What you will do- Provide professional nursing services under the direction of a RN in compliance with the state s Nurse Practice Act, licensure/certification requirements, applicable healthcare standards, laws, and Center Well Home Health policies.
- Offer skilled nursing interventions for illness, rehabilitative needs, and preventative care; perform teaching and training activities per the plan of care.
- Apply clinical standards to facilitate problem resolution and achieve patient goals and outcomes.
- Confer with the RN Supervisor regarding changes to the Plan of Treatment; accept verbal orders where permitted by law and communicate these orders to the RN Supervisor.
- Use a systematic, individualized, goal/outcome-driven approach to implementing the nursing plan of care.
- Maintain contact with patients, physicians, clinical managers and other team members regarding needs and status changes; participate in care coordination and discharge planning as appropriate.
- Maintain professional conduct and confidentiality of patient information, sharing only as legally required and in the patient s best interest.
- Ensure availability of supplies, materials and equipment needed to safely implement the plan of care.
- Prepare, submit and maintain documentation as required; visit/shift notes for day services rendered.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver s license, auto insurance and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
- Pay per visit/unit: $28.00 - $40.00
- Base pay: $44,600 - $61,400 per year
- Scheduled weekly hours: 1
