Job Summary

We are currently looking to fill a casual opportunity for Registered Nurse/Registered Psychiatric Nurse for Crossroads Hospice in Port Moody, BC.

Crossroads Hospice is a 168‑bed hospital in Port Moody, British Columbia, serving the communities of Coquitlam, Port Coquitlam, Port Moody, Anmore and Belcarra. The hospital offers medical, surgical, and cardiac programs, as well as 24/7 emergency services with over 53,000 visits annually.

Detailed Overview

As a member of a multidisciplinary team, you will provide direct nursing care to patients of the Hospice Residence at assigned locations according to established policies and procedures by assessing physical, emotional, spiritual and psychosocial needs; planning, documenting and implementing care plans; evaluating the effectiveness of the care plan and making recommendations for care plan changes when necessary.

Responsibilities
  • Assess, plan, implement and evaluate nursing care of assigned patients by assessing actual or potential problems and strengths, planning and performing interventions and evaluating outcomes.
  • Collect information about the patient using a variety of sources such as skills of observation, communication and physical assessment to enable appropriate nursing care.
  • Communicate and consult with members of the health care team about the patient care plan; participate in interdisciplinary conferences and explain plan of care to the patient and/or families.
  • Act as an advocate to protect and promote the patient's rights to autonomy, respect, privacy, dignity and access to information.
  • Document timely and accurate reports of relevant observations including patient and family teaching and evaluation of nursing care.
  • Participate in nursing, hospital committees and councils for improvement of patient care and/or work environment.
  • Participate in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor where appropriate; identify self‑learning needs and attend educational programs to maintain and enhance clinical competency.
  • Participate in quality improvement activities by identifying patient care issues and collecting data, evaluating patient care programs, techniques, standards and procedures by providing feedback about effectiveness and patient care outcomes.
  • Maintain a safe environment for patients, families and staff by adhering to established safety and emergency practices and procedures.
  • Perform other related duties as assigned.
  • Qualifications

    Education and Experience

    Graduation from an approved School of Nursing and/or Psychiatric Nursing. Two (2) years' recent, related medical nursing experience in an acute care facility, including one (1) year recent related experience in a hospice/palliative care environment, or an equivalent combination of education, training and experience.

    Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
