Position: Community Outreach Worker - Flexible Hours

Connective is seeking a Casual Outreach Worker dedicated to enhancing community integration for clients. As a part of our valued, unionized team, you’ll assess client needs, devise supportive plans, and engage with multiple stakeholders. Prior experience in social services and a commitment to supporting vulnerable populations are ideal for this role.

Key Responsibilities:

• Assess client needs and gather background information

• Develop intervention and support plans with measurable goals

• Address daily living challenges including housing and nutrition

• Support clients in diverse environments and situations

• Collaborate with professionals and families for best outcomes

Requirements:

• Diploma in Social Services preferred

• One year of relevant experience or in-house training available

• Crisis management skills with vulnerable populations

• Valid driver’s license and personal transportation

• Ability to complete criminal records and reference checks
