About the Role
Our client is seeking a dedicated and skilled Community Health Nurse to serve vulnerable populations in Port Elizabeth . In this vital role, you will provide essential healthcare services, health education, and support directly within the community, focusing on improving health outcomes for underserved individuals and families. You will work proactively to address health disparities, promote preventative care, and connect clients with necessary medical and social services.
This position requires a compassionate approach, strong clinical skills, and the ability to build trust and rapport within diverse community settings. Your efforts will directly contribute to the health and well-being of the community.
- Conduct health assessments, screenings, and basic medical treatments for individuals and families in community settings.
- Provide health education on topics such as chronic disease management, maternal and child health, and infectious disease prevention.
- Administer immunizations and prescribed medications as per protocols.
- Facilitate referrals to physicians, specialists, and other healthcare and social service providers.
- Maintain accurate patient records and contribute to program data collection for reporting and evaluation.
- Collaborate with community leaders and organizations to identify health needs and develop targeted interventions.
- A Registered Nurse (RN) qualification.
- Current registration with the South African Nursing Council (SANC).
- Minimum of 3 years of experience in community health nursing or a related field.
- Strong clinical assessment and patient care skills.
- Excellent communication, interpersonal, and cross-cultural competency.
- Knowledge of public health principles and community resources in the Port Elizabeth area.
