Case Manager

Our client is seeking a diligent and organized Case Manager to join their dedicated team in Port Elizabeth . This role is central to assisting individuals and families in accessing and utilizing essential social services and support systems. You will be responsible for assessing client needs, developing tailored action plans, and advocating for their well‑being. This position requires hands‑on community engagement and a commitment to empowering clients towards self‑sufficiency and improved quality of life.

• Conduct thorough assessments of client needs, circumstances, and goals.
• Develop, implement, and monitor comprehensive case management plans.
• Connect clients with relevant community resources, including healthcare, housing, employment, and social programs.
• Provide ongoing support, advocacy, and guidance to clients throughout their journey.
• Maintain accurate and detailed case records, ensuring confidentiality and compliance.
• Collaborate effectively with a network of service providers and stakeholders in Port Elizabeth .

• Relevant degree or diploma in Social Work, Community Services, or a related field.
• Proven experience as a Case Manager or in a similar client support role.
• Strong knowledge of social welfare systems and community resources in the Eastern Cape.
• Excellent assessment, planning, and problem-solving skills.
• Effective communication and interpersonal abilities.
• Ability to manage a caseload and work independently in the field.
