Are you an experienced clinician passionate about transforming care for people living with frailty? We are seeking an enthusiastic, experienced, and forward-thinking Advanced Frailty Practitioner to join our developing frailty service in Inverclyde. This is an exciting opportunity to support the development of an integrated approach to frailty, rehabilitation, and care closer to home, helping people maintain independence, prevent functional decline, and avoid unnecessary hospital admissions.

The post holder will provide advanced clinical assessment, clinical reasoning, and decision making for people with complex needs, working autonomously and as part of a multidisciplinary team to deliver person-centred care and rehabilitation.

Duration, Location, and Working Pattern:

Permanent - 36 Hours

Centre for Independent Living, Greenock

5 days over 7, including weekends/public holidays

Key responsibilities:

The Advanced Frailty Practitioner will work across health and social care services to provide expert assessment, treatment, rehabilitation planning and care coordination for frail older adults with complex needs. The post holder will work closely with Geriatric Medicine, District Nursing, Care Home Liaison Nursing, Allied Health Professionals, Social Work, Care Homes, Primary Care and wider community services to deliver integrated care.

Responsibilities include facilitating Comprehensive Geriatric Assessment (CGA), developing personalised care and rehabilitation plans, undertaking complex clinical decision-making, supporting admission avoidance initiatives, and promoting the Right Person, Right Time, Right Place approach. The role will contribute to service development, quality improvement, clinical governance, research and innovation, while providing specialist education, support and leadership to healthcare professionals, patients, carers and families. The post holder will also support the development of the frailty hub and spoke model, working collaboratively across organisational boundaries to enhance frailty pathways, early intervention, rehabilitation coordination and future care planning.

Knowledge,

training, qualifications and/or experience required to do the job:

Registered Nurse with current NMC registration or Allied Health Professional with significant post-registration experience
Substantial experience working with frailty, older adult care and/or unscheduled care
Master's level qualification in Advanced Practice, or working towards this qualification
UK driving licence is essential for the role, with access to a vehicle for work purposes.
Evidence of competence in advanced clinical assessment
Advanced clinical reasoning, judgement and diagnostic decision making skills
Knowledge of anatomy and pathophysiology
Non-medical Prescribing qualification (V300) desirable/essential as per local requirements
Experience managing patients with complex health and social care needs
Expert knowledge of frailty assessment and management
Experience facilitating Comprehensive Geriatric Assessment (CGA)
Strong multidisciplinary team working and care coordination skills
Knowledge of NHS policies, professional standards, procedures and clinical guidelines
Evidence of service improvement, clinical leadership and professional development
Experience delivering education, presentations and training
