Position: Full-Time Social Worker
Enhance residents' lives at Seaforth and Queensway Long Term Care as a full-time Social Worker in Port Hope, Ontario. Provide essential emotional and practical support for residents and their families while promoting dignity and independence.

This permanent full-time role focuses on supporting two care homes biweekly, addressing psychosocial needs, and ensuring compliance with relevant legislation. You will facilitate care conferences, offer counseling, and connect families with community resources, all while cultivating a caring, resident-centered environment.

Key Responsibilities:

• Coordinate interdisciplinary care conferences for new admissions

• Develop programs for psychosocial resident support

• Provide advocacy and emotional support for families

• Lead staff education on social work topics

• Maintain confidential documentation per legal standards

Requirements:

• Bachelor/Master of Social Work (BSW/MSW) or equivalent

• Current registration with the Ontario College of Social Workers

• Experience in long-term care or gerontology preferred

• Strong interpersonal and communication skills

• Empathy in managing sensitive situations
