Per Diem Hospice and Palliative Nurse Practitioner
We are looking for a compassionate and experienced Nurse Practitioner to join our hospice and palliative care program. The ideal candidate holds a Michigan license and is dedicated to providing high‑quality, patient‑centered care.
Qualifications- Current unrestricted Nurse Practitioner in Michigan (required)
- Minimum of one year experience in a Medicare‑certified home health or hospice agency (preferred)
- Demonstrated knowledge of home health nursing, hospice, and/or outpatient services (preferred)
- Willingness to meet the agency need to support hospice Medical Director/designee
- Understanding of performance improvement with the ability to communicate and operationalize initiatives at the departmental and organizational level
- Excellent interpersonal skills, sound judgment, effective organization, prioritization, and follow‑through; attention to detail, tact, dependability, emotional intelligence; ability to maintain confidentiality and promote positive, constructive relationships with communication and collaboration at all levels (required)
- Positive, professional interpersonal skills (required)
- Meets health requirements demanded of all clinical/field staff who provide service through the agency
- Perform routine and urgent assessments; available PRN/Part Time
- Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
- Order, perform, and interpret laboratory and radiology tests within scope of practice to assess patient’s clinical problems and health‑care needs
- Prescribe medications including controlled substances to the extent delegated and licensed
- Order treatments and durable medical equipment as indicated
- Perform other therapeutic measures as indicated
- Assist patients and families in identifying goals of treatment and ongoing plans of care at every visit; communicate these goals among the Interdisciplinary Team
- Consult with palliative care physician or designee as needed; inform primary physician of services provided and collaborate with other physicians as necessary
- Assist in all facets of care coordination for palliative care referrals
- Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
- Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
- Participate in Quality Assurance activities and committees related to Nurse Practitioner scope of practice and policies/procedures
- Comply with applicable laws and regulations with respect to Collaborative Agreements
- Provide training and ongoing education and onboarding support for staff
- Define goals for professional growth and participate actively in professional activities and organizations
- Engage in active and frequent self‑care activities for personal and professional growth and longevity
- Adhere to the practice of confidentiality regarding patients, families, staff, and the Organization
- Collaborate and communicate with Medical Director, attending physician, hospital staff, and Interdisciplinary Team; participate in Interdisciplinary Team meetings bi‑weekly
- Collaborate/communicate with Interdisciplinary Team to meet patient needs for clinical and psycho‑social interventions; assist in identifying the need for intervention of other members; effectively communicate patient and family needs to the Team
- Complete and submit required clinical documentation within 24 hours of visit completion
- Educate patient and caregiver regarding:
- Care of patient
- Disease process
- Goal setting
- Symptom control
- Treatment options
- Prognosis
- Advance Care Planning
- Supervise nurse trainees, including graduate and advanced practice nurses in field placements
- Must be able to operate an automobile/has access to personal transportation
- Able to lift/transfer/push/pull up to 100 pounds infrequently using assistive techniques and devices appropriate to manage the task
- Must be able to complete multi‑tasks and meet multi‑deadlines
- Proactively promote census growth and seek to offer care that meets the unmet diverse needs of the community
