Licensed Practical Nurse (LPN)

A full‑time LPN role at Yes Care in Port Huron, MI. Entry level, Health Care Provider, Hospitals and Health Care industry.

$31.50/hr

• Hours 6:00 AM – 6:30 PM and 6:00 PM – 6:30 AM, 12‑hour shifts
• Shift differentials and holiday pay

• Patient Assessment and Care Planning:
  Assist in assessing patients’ health status and contribute to individualized care plans.
• Direct Patient Care:
  Provide safe, effective nursing care for multiple patients within the facility’s program.

  Implement patient education initiatives based on established teaching plans.
• Nursing Scope of Practice:
  Perform delegated tasks consistent with the LPN scope of practice.
• Clinical Decision Making:
  Use problem‑solving to make informed clinical decisions.
• Care Coordination and Monitoring:
  Oversee on‑site and off‑site patient scheduling with the care team.
• Follow‑up Care:
  Ensure follow‑up care, including telemedicine appointments and consult processes.
• Quality Improvement:
  Participate in QI initiatives and Patient Care Committee meetings.
• Leadership
  
  Collaboration:
  
  Coordinate with Regional Medical Director and other leadership to ensure seamless patient care.
• Documentation:
  Monitor CARES and coordinate as needed; perform additional duties as assigned to support patient outcomes.

• Educational Background:
  Graduate of an accredited Licensed Practical Nursing program.
• Licensure and Certification:
  Current LPN licensure in the state of employment and active CPR certification.
• Skills and Knowledge:
  Familiarity with state laws governing LPN practice and successful completion of initial and annual clinical competencies.
