Clinical Pharmacist, Correctional Health Services – Fraser Health Community North Fraser (Tri‑Cities / Coquitlam / Port Coquitlam / Port Moody / Anmore / Belcara)

Fraser Health Authority is recruiting a full‑time Clinical Pharmacist for its Correctional Health Services team. Salary
: CAD $51.37 – $64.19 per hour.

Job Summary

We are a collaborative network of four premier health employers—Fraser Health, Provincial Health Services, Providence, and Vancouver Coastal Health—providing integrated pharmaceutical care across our correctional facilities. The Clinical Pharmacist will work directly with patients to develop and monitor therapeutic plans, provide medication counseling, and collaborate with physicians and other health professionals.

  • Ensuring prescriptions are validated, dispensed accurately, and medication safety concerns are resolved expeditiously
Responsibilities
  • Identify and resolve drug therapy problems in collaboration with the client and healthcare team.
  • Provide comprehensive medication management and pharmaceutical care by monitoring effectiveness, adherence, adverse reactions, drug interactions, and laboratory incompatibilities.
  • Consult with clients and healthcare team to develop client‑specific therapeutic plans based on chart review and assessment of clinical responses.
  • Maintain documentation in the health record in accordance with organizational standards.
  • Use appropriate resources, such as provincial programs, academic detailing, opioid stewardship, de‑prescribing initiatives, to support professional practice.
  • Establish linkages with health authority and community pharmacy services to support continuity of care.
  • Provide drug information, consultation, and support services to other healthcare team members.
  • Conduct client counseling and education regarding drug therapy.
  • Provide in‑service education to the healthcare team when appropriate.
  • Offer instruction and supervision to students, including undergraduate pharmacy students and residents.
  • Participate in committees and provide guidance on drug therapy.
  • Engage in quality improvement activities, reviewing current practice and recommending modifications.
  • Maintain workload records and metrics as required.
  • Contribute to programs such as Patient Safety and Drug Reaction Reporting.
  • Help orient new personnel and model professional behaviors.
  • Maintain a working knowledge of Pharma Care policies and procedures.
  • Undertake related duties as assigned.
Qualifications

Education and Experience

  • Current registration as a pharmacist with the College of Pharmacists of British Columbia.
  • Two (2) years of recent, relevant patient care experience, or an equivalent combination of education, training, and experience.
  • Valid BC driver's licence and access to a personal vehicle for business‑related purposes.

Skills and Abilities

  • Broad knowledge of organizational policies, procedures, and standards of care.
  • Knowledge of evidence‑based clinical pharmacy practice.
  • Experience in medication use evaluation.
  • Familiarity with research processes and methodology.
  • Understanding of drug distribution processes in a health system setting.
  • Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision.
  • Strong verbal and written communication skills.
  • Ability to establish and maintain effective relationships with healthcare practitioners, clients, family members, and the public.
  • Organizational and prioritization skills.
  • Proficiency in operating related equipment.
  • Physical ability to perform the duties of the position.
