Detailed Overview

Coordinates, screens and prioritizes hospice palliative client activities by assessing clients, consulting with the interdisciplinary team and coordinating client care across the continuum of care; acts as a clinical resource to other nurses and team members by providing clinical guidance and/or nursing consultation based on specialized theory, knowledge, education, and practice. Intervenes in client care provided by other nurses and/or healthcare providers, as required, to ensure that client outcomes are consistent with client care plans and the delivery of quality client care;

implements, evaluates and recommends clinical education plans for nurses and other staff.

Responsibilities
  • Supports the interdisciplinary team by monitoring standards of care and acts as a clinical nurse resource; assesses, demonstrates, and directs the provision of care provided to the client, role‑modeling, coaching, and mentoring team members in therapeutic communication.
  • Coordinates and prioritizes client care activities by providing formal clinical consultations to nurses, interdisciplinary team members and students; reviews client needs to troubleshoot and provide guidance in care management.
  • Consults with the interdisciplinary team and other healthcare providers in resolving client care issues; provides feedback and assists nursing staff with critical thinking skills.
  • Coordinates development and implementation of clinical education, training, and orientation for the interdisciplinary team to identify learning requirements and update learning plans with staff.
  • Assesses individual and interdisciplinary team clinical skills by reviewing competencies, practice changes, and quality improvement indicators; designs, develops, implements, and evaluates clinical education programs.
  • Acts as a preceptor, clinical resource and mentor to nursing and other staff; provides evidence‑based teaching and training to staff.
  • Coordinates and participates in quality improvement and risk management activities; evaluates changes to client care practice and conducts safety audits.
  • Participates in interdisciplinary rounds and care conferences; acts as a client advocate for dignity, autonomy, respect, privacy, and access to information.
  • Participates in selection of new staff and performance evaluation; attends panel interviews and assesses clinical skills.
  • Promotes the use of research in nursing; searches for information and implements applications of current research findings.
  • Sets measurable goals and objectives to maintain clinical competence and professional development.
  • Performs other related duties as assigned.
  • Qualifications

    Education and Experience

    Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives. Three (3) years of recent related clinical nursing experience in the designated clinical area, including one (1) year of supervisory or leadership experience, or an equivalent combination of education, training, and experience. Completion of the applicable Canadian Nursing Association certificate in the designated clinical area.

    Valid BC Driver’s License and access to a personal vehicle for business‑related purposes.
