Position: Health Care Nurse for Community Support
Join Home Health as a Community Health Nurse in Port Moody, B.C., providing essential support to clients. This relief full-time role focuses on chronic disease management and client transitions to enhance well-being in the community.

In this role, you will leverage your nursing skills to perform assessments, create care plans, and collaborate with healthcare teams. Your ability to manage complex patient needs and promote health will directly impact clients' home environments. Foster lasting relationships as you guide clients and their families through their healthcare journeys.

Key Responsibilities:

• Screen patient referrals and perform comprehensive assessments

• Develop and maintain care plans in collaboration with teams

• Directly provide in-home client care as needed

• Advocate for clients’ health needs and preferences

• Organize client care conferences with healthcare professionals

Requirements:

• Registration with the British Columbia College of Nurses and Midwives

• Minimum one year of experience in geriatric and chronic illness care

• BC Driver's license required for travel to clients

• Ability to prioritize patient care and communicate effectively

• Strong skills in collaboration within interprofessional healthcare teams
