This Full Scope Licensed Practical Nurse role is designed for individuals passionate about patient care and community health. You will independently manage client assessments while promoting health restoration and maintenance strategies. The role encompasses patient education, care planning with interprofessional teams, and ensuring timely support for chronic health conditions.

Key Responsibilities:

• Screen client referrals and conduct assessments

• Develop shared care plans with team members

• Advocate for client choices during transitions

• Provide health counseling and wellness promotion

• Maintain records in accordance with policy

Requirements:

• Current registration as a Licensed Practical Nurse in B.C.

• Minimum of one year experience in outpatient settings

• Access to a vehicle for client visits

• Knowledge of gerontology and chronic illness management

• Capacity to thrive in dynamic healthcare environments
