We are seeking a relief full-time Licensed Practical Nurse to enhance the quality of life for patients. In this role, you will establish therapeutic relationships, develop care plans, and collaborate with an interprofessional team. Key Responsibilities:

• Establish therapeutic relationships with clients through effective communication

• Conduct client assessments and develop individualized care plans

• Collaborate with healthcare teams for coordinated care

• Supervise Community Health Workers in client care tasks

• Provide health education and self-management support

Requirements:

• Current registration as a Licensed Practical Nurse in B.C.

• Minimum 1 year of clinical experience with chronic health patients

• Knowledge of community health services and resources

• Excellent communication skills, verbal and written

• Valid BC Driver's license and access to a vehicle
