Community Liaison Nurse

Position: Community Liaison Nurse at Fraser Health
As a Registered Nurse/Registered Psychiatric Nurse, take on a crucial role in community health with Fraser Health in the Tri-Cities area. Engage in effective mental health care and crisis intervention to improve patient recovery.

This Full Time position requires you to work closely with interdisciplinary teams to facilitate patient transitions from hospitals to community-based services. You’ll provide crucial assessments, referrals, and follow-up treatment aimed at supporting children and adolescents facing mental health challenges. Your efforts will greatly reduce hospital bed usage.

Key Responsibilities:

• Collaborate on patient discharge plans with the healthcare team

• Conduct assessments and crisis intervention for clients

• Refer patients to community resources for ongoing support

• Keep comprehensive client records as required

• Engage in educational programs for continuous learning

Requirements:

• Graduation from an approved nursing program

• Active registration as an RN or RPN with BCCNM

• Two years of related psychiatric experience

• Class 5 BC Driver's License

• Strong organizational and communication skills
