Fraser Health seeks a Registered Nurse/Registered Psychiatric Nurse to improve community mental health connections in the Tri-Cities, B.C. Focus on providing essential crisis support and recovery services.
As a Full Time nurse, you will be instrumental in assessing clients' needs and supporting transitions from hospital to community-based care. This role emphasizes recovery-oriented practices for children and adolescents, making your contributions vital in crisis situations and crisis prevention. Your ability to work effectively within a multidisciplinary team is crucial for success.
Key Responsibilities:
• Collaborate with healthcare team on patient discharges
• Assess and provide counseling for at-risk clients
• Ensure referrals to appropriate community services
• Document client information in clinical systems
• Attend meetings and contribute to quality initiatives
Requirements:
• Graduation from a recognized Nursing program
• Current RN/RPN registration with BCCNM
• Minimum two years of experience in similar roles
• Class 5 BC Driver’s License and vehicle access
• Understanding of mental illness treatment
