Fraser Health Community Nurse Opportunity

We are looking for a Community Health Registered Nurse to fill a casual position at Fraser Health. In this entry-level role, you will work independently and as part of a team to manage client caseloads while providing assessments, coaching, and interventions. Your efforts will contribute to the quality of life for patients in the community.

Key Responsibilities:

• Screen referrals and perform client health assessments

• Develop interprofessional care plans for complex conditions

• Provide direct care and client education in various settings

• Coordinate joint home visits with other healthcare professionals

• Maintain clinical records in line with policy

Requirements:

• Registered Nurse certification with BCCNM

• One year of recent experience in community/outpatient care

• Access to a vehicle for client visits

• Demonstrated competence in community health

• Strong problem-solving and organizational skills
