Casual Social Worker

Join Fraser Health as a Casual Social Worker with Hospice Palliative Community Services in Port Moody, BC. This role requires expertise in psycho-social assessments and therapeutic counseling for palliative care clients.

As a Casual Social Worker, you will provide critical social work services for clients and their families dealing with end-of-life issues. Collaborating with an interdisciplinary team, you will conduct assessments, create care plans, and ensure clients access necessary community resources. This role demands sensitivity, clinical skills, and the ability to manage complex psychosocial situations effectively.

Key Responsibilities:

• Conduct social and psychosocial assessments of clients

• Provide grief and bereavement counseling services

• Collaborate with interdisciplinary care teams

• Facilitate client access to community resources

• Participate in quality improvement activities

Requirements:

• Master’s degree in Social Work

• Two years of clinical experience with palliative clients

• Full registration with the BC College of Social Workers

• Valid BC Driver's license and vehicle access

• Strong counseling and assessment skills
