Overview
Envision Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Kitsap County, WA. The role provides care in private homes, assisted living, and skilled nursing facilities. This opportunity includes a $10,000 sign-on bonus. We offer tailored hospice training, 1:1 nurse-to-patient ratios, clinical autonomy, and a supportive, collaborative culture with ongoing opportunities for professional growth and development.
Responsibilities- Conduct initial and ongoing assessments to address physical, emotional, and spiritual needs and develop individualized care plans.
- Collaborate with physicians, social workers, volunteers, and other interdisciplinary team members to implement and manage care plans.
- Administer medications and interventions to manage pain and symptoms, ensuring patient comfort and quality of life.
- Educate patients and families on the hospice process, symptom management, self-care, and end-of-life issues.
- Provide counseling and support to patients and families dealing with grief, anxiety, and other concerns.
- Maintain accurate, timely, and compliant patient records reflecting care provided and changes in condition.
- Ensure care delivery aligns with Medicare, state regulations, and hospice standards.
- Promote patient dignity, preferences, and rights in all aspects of care.
- Respond promptly to urgent needs and coordinate with the on-call team for additional support.
- Participate in ongoing quality assurance and performance improvement initiatives.
- Perform additional tasks as assigned, including after-hours responsibilities.
- Education & License:
Active RN license required; hospice/palliative care certification (e.g., CHPN) a plus. - Experience in hospice or palliative care settings is preferred, with a strong understanding of end-of-life care principles and best practices.
- Strong clinical assessment, communication, and interpersonal skills with a compassionate, patient-centered approach.
- Certifications:
BLS required; ACLS preferred (based on care setting). - Technical Proficiency:
Proficient with EHR systems and healthcare technologies. - Other Requirements:
Ability to work independently, respond to crises, and adapt to diverse social, cultural, and religious backgrounds. Must have reliable transportation, a valid driver’s license, and auto liability insurance.

Compensation: $104,000 - $118,000 per year, depending on experience. All rates are negotiable within this range and will be discussed during the hiring process.
