Registered Nurse – Home Health – Full-Time

• Evaluate patients to determine appropriateness for home health care
• Develop and implement patient‑specific care plans with appropriate frequency of care
• Communicate with the care team, physicians, payors, patients and families to address care needs and fulfill the care plan
• Deliver high‑quality skilled care and supervise the care of other team members
• Document care delivery daily in the EMR system
• Contribute to a culture of accountability and teamwork

• Current RN license, valid state driver’s license, and reliable transportation
• One year of experience as a RN in an acute or clinical setting preferred; home health experience a plus

• Compassionate care delivery focused on results
• Solution‑driven, self‑motivated, and responsive with urgency
• Inspiring patients to reach their goals
• High level of RN license competency
• Comfortable with technology; HCHB experience is a plus
