Immediate Licensed Practical Nurse (LPN) - Home Health opportunity in Portage, MI. Territory:
Calhoun/Branch counties.

Pay Rate:

• $38.00/hr - local candidate pay rate
• Travel pay package available
• Mileage Reimbursement: $.49/mile for distance covered between patient homes

Work Schedule:

• 9:00am - 5:00pm (Monday to Friday)
• 40 hours per week
• No weekends
• 13-week contract

Job Details:

• Administer skilled nursing care and education under the supervision of registered nurse for adult clients in their place of residence, coordinate with the interdisciplinary team, patient/family and provider.
• Support patient recovery and ability to stay at home – enabling them to spend quality time with their loved ones and doing the activities they enjoy
• Empower patients and families to reach healthcare goals by educating them on disease management
• Case manage and provide clinical oversight to interdisciplinary team

Qualifications

• Current LPN License
• Must have at least 12 months of home health experience within the past 3 years.
• Hands-on HCHB experience required
• Current driver’s license
• American Heart Association BLS
