Sierra Valley Rehab Center, a premier skilled nursing and rehabilitation facility in Porterville, CA, is seeking a passionate and licensed Physical Therapist to join our dedicated therapy team. Position Summary:

The Physical Therapist will evaluate and treat patients as prescribed by a physician and will play a key role in the development and implementation of care plans. This position collaborates with the interdisciplinary care team and supervises PTAs and students, following all regulatory and professional standards.
Key Responsibilities:

• Evaluate patients within 24 hours of physician referral (or document delay appropriately)
• Develop individualized treatment plans and obtain physician approval
• Provide therapy in alignment with established treatment plans
• Supervise Physical Therapy Assistants and aides as per state regulations
• Participate in care conferences, in-services, and interdisciplinary meetings
• Document evaluations, daily notes, progress, and recertifications according to payer and legal requirements
• Educate patients, caregivers, and nursing staff on proper therapy techniques and discharge planning
• Recommend and assist with ordering durable medical equipment as needed
• Maintain cleanliness and functionality of therapy equipment
• Assist with restorative aide training and general staff education

• Bachelor's, Master's, or Doctorate in Physical Therapy
• Current Physical Therapist license in California (in good standing)
• CPR certification (or obtained upon hire)
• Proficient in documentation systems (e.g., Casamba, Point Click Care )
• Experience in a SNF or post-acute setting preferred
• Strong problem-solving and communication skills

• Frequent standing, walking, pushing, pulling, and reaching
• Occasional lifting of 100+ lbs with assistance
• Must be able to perform physical tasks associated with patient mobility and rehab care
