* Key Responsibilities:

*** Provide direct nursing care to residents in accordance with individualized care plans
* Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care
* Administer medications and treatments as prescribed
* Monitor residents for changes in condition and report to RN Supervisor or physician
* Document all care provided accurately and timely in EMR system
* Communicate effectively with residents, families, and interdisciplinary team
* Maintain compliance with safety, infection control, and facility policies
* Respond to emergencies and follow established protocols
* Participate in shift handoffs, training, and care planning###

* Qualifications:

**
* ** Current California LVN license
** in good standing
* CPR certification
* Experience in a SNF or post-acute setting preferred, but
** not required
*** New graduates encouraged to apply –

*** Strong written and verbal communication skills
* Computer literate and comfortable with EMR systems
* Compassionate, dependable, and a team player
* Must be able to provide recent medical clearance to work in SNF
* Must pass background check and be eligible to work in the U.S.###
