** Requirements:
*** 2 years LVN experience, home health preferred
* LVN license, required
* IV Therapy/Blood Draw certification required
* Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
* Experience with Kinnser (Well Sky) EMR preferred
* Experience with Allscripts preferred
* Reliable transportation required
** Primary responsibilities will be:
*** Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s residence.
* Follow the patient Plan of Care (POC) as directed by RN Case Manager and PCP
* Communicate necessary revisions to patient status and/or needs to RN Case Manager and Clinical Manager when applicable
* Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.
* Provide complete, accurate and timely documentation consistent with the plan of care
