The Clinical Case Manager will work in our supervised apartment program for women in the Middletown area. The role includes case management, advocacy, educational, and supportive services on a schedule of T, W, Th, F, Sa 7:00 a.m.‑3:00 p.m. The individual will provide all phases of clinical services—including intake, engagement, assessment, treatment planning, case management, referral, and discharge—while completing timely documentation.

• Bachelor’s degree in a human service field (Master’s preferred)
• At least one year of clinical experience in trauma, co‑occurring psychiatric or substance abuse treatment
• Valid driver’s license and automobile insurance in good standing
• Reliable personal vehicle for service visits
• Experience with trauma treatment and co‑occurring disorders (preferred)

Union position: $18.89‑$20.69 per hour, depending on education level.

• Provide clinical knowledge regarding severe mental health and substance abuse disorders through training and education.
• Collaborate with the treatment team to manage clients’ mental health and assess risks, implementing risk management plans as needed.
• Deliver recovery‑oriented clinical case‑management services with dignity, respect, and inclusion of family and natural supports.
• Offer skills training in daily living, nutrition, financial management, and symptom management.
• Monitor outcomes and ensure program compliance through accurate and timely data reporting.
• Complete assessments, treatment plans, and documentation electronically and on paper in a timely manner.
• Act with compassion, integrity, and ethics.
