This position will be traveling to see patients in the East Portland Metro area.

• Provide patient-focused, compassionate care in home health, hospice or related settings as appropriate.
• Travel to patient locations within the East Portland Metro area to deliver care.
• Collaborate with the care team to support the patient’s health goals and comfort.

• Graduate of an accredited LPN/LVN school of nursing
• Within 30 days of hire:
  National Provider BLS - American Heart Association
• Upon hire:
  Oregon Licensed Practical Nurse
• Washington Licensed Practical Nurse (within 60 days of hire)
• Driving is a necessary part of this role. Caregivers must comply with all state laws and requirements for driving and provide proof of driver license and auto insurance upon request.
• Upon hire:
  Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
• Upon hire:
  Criminal background check compliant with Providence St. Joseph Health - OR policies
• At least 1 year of nursing experience in long-term care, acute care, ambulatory care or home health and hospice care

• Experience in home health, hospice or public health
