  • Provide field-based outreach, engagement, and crisis intervention
  • Complete mental health assessments and short-term therapeutic interventions
  • Facilitate interdisciplinary case conferences and care planning
  • Coordinate care across medical, housing, crisis, and community systems
  • Offer bridge services while clients await ongoing behavioral health treatment

Location: Golden West

Schedule: Monday-Friday 8:30am-5:00pm

Salary Ranges from: $65,000- $113,000
* This range is influenced by professional experience, licensure/certifications, and internal equity.

Must meet qualifications for:
    • QMHP or QMHP-R
    • LPC-A or LPC
    • LMFT-A or LMFT
    • CSWA or LCSW
  • LCSW / LPC HIGHLY Preferred.
  • CADC-R, at minimum, preferred.
  • Experience providing care coordination, case management, or other services to an under-served adult population experiencing mental illness, substance abuse, and/or houselessness preferred.
  • Has the competencies necessary to:
    • Identify precipitating events
    • Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts
    • Assess family, social and work relationships
    • Conduct a mental status assessment
    • Document a DSM diagnosis
    • Write and implement or supervise implementation of a treatment plan
    • Conduct and document a mental health assessment
    • Provide mental health treatment and rehabilitative services within one's scope of practice
  • Current CPR certification required prior to start.
  • Must meet CCC privileging requirements as required by FTCA.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Experience working with dually-diagnosed individuals preferred.
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
  • Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit.
  • Must be able to work variable hours.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
  • Must adhere to agency's non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles and sexual orientations, and treat individuals with respect and dignity.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Provide timely outreach, engagement, and brief solution-focused crisis interventions to CCC clients who are identified as needing enhanced behavioral health supports.
  • Maintain a case load of 20 individuals who are identified as needing increased behavioral health supports in order to maintain their housing and voucher.
  • Participate in SPC team meetings and huddles, and provide consultation as well as subject matter expertise regarding the behavioral health of all clients in the CCC SPC program.
  • Coordinate with SPC supervisor regarding the prioritization of CCC clients identified as at risk for housing instability or lease enforcement for a reason related to their behavioral health.
