PTA - Physical Therapist Assistant

This is a 13-week contract position.

Day shift, 5x8-Hour (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s. Weekend work is not required.

We are seeking a dedicated Home Health Physical Therapy Assistant (PTA) to provide quality care to patients in their homes. This role includes working 1:1 with patients and may involve visiting up to 6 patients per day. The position requires efficient management of time to meet the productivity standard of 30 points per day.

• Provide skilled therapy services to patients in their homes.
• Follow care plans as developed by the supervising physical therapist.
• Maintain effective communication with patients, family members, and the healthcare team.
• Document patient progress using EPIC and Oasis charting systems.
• Travel to patient homes, managing a maximum radius of roughly 60 miles per day.

• Minimum of 1 year of skilled home health experience.
• Current BCLS certification is required.
• Oasis experience is required.
• Ability to provide proof of supervision references.
