Physical Therapist (PT) – Home Health

East Portland, OR & Surrounding Areas
13-Week Contract | Full-Time | Day Shift

Schedule

• Shift: Day Shift | 5x8 (8:00 AM – 4:30 PM)
• Start Date: 02/16/2026 (ASAP)
• Weekend Requirement: None
• On-Call: No
• Guaranteed Hours: Facility may call off up to 1 shift per pay period

Key Responsibilities

• Perform initial evaluations, reassessments, and discharge planning
• Develop and implement individualized plans of care
• Provide skilled PT services including:
  • Gait training and mobility restoration
  • Post-operative orthopedic rehab (THR, TKR, spinal, shoulder)
  • Neurologic rehab (Stroke, Parkinson’s, MS, ALS, Dementia)
  • Wound care and fall prevention
  • Cardiopulmonary rehab (CHF, COPD, CABG)
• Complete accurate OASIS documentation
• Educate patients and caregivers on home exercise programs and safety
• Collaborate with interdisciplinary care teams

Requirements

• Active PT license (pending accepted)
• Minimum 1 year of skilled Home Health PT experience
• OASIS experience REQUIRED
• BLS certification
• Ability to work independently in a home health setting
• Reliable transportation required (mileage reimbursed)
