Department Overview

Family Medicine at Gabriel Park is a community clinic located in SW Portland. We serve our patients in varying stages of life and throughout their lives. Our services include general family medicine, transgender care, maternity care (including deliveries), pediatric care, behavioral health, sports medicine, travel immunizations, acupuncture, a retail pharmacy, x‑ray, minor procedures, and health maintenance with focused preventive care. We are a high‑volume clinic that sees up to 250 patients and receives 400‑600 phone calls daily.

Function/Duties of Position

The OHSU Clinical registered nurse (RN) provides compassionate, evidence‑based, and efficient care to individuals, families, communities, and patient populations. The Clinical RN’s care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The Clinical RN demonstrates the professional role obligations of scientist, leader, and knowledge transferor (Onsobble Model of the Professional Role™).

Description of work:

Provides effective care coordination for an assigned panel of approximately 200 high‑risk patients with chronic conditions including (but not limited to) congestive heart failure, hypertension, diabetes, chronic obstructive pulmonary disease, as well as those with complex psychosocial needs or receiving home health or hospice/end of life care. Responsible for the active management, including transition management, of patients as the liaison between referral providers and other members of the patient’s care team.

Participates in clinic, department, OHSU campus, teams and committees. Expected to initiate and/or actively participate in quality improvement projects as part of a multi‑disciplinary team, working with providers, medical assistants, lab personnel, and patient access services. Assists in the implementation of a nursing system that supports the mission of the American Academy of Ambulatory Care Nursing (AAACN).

Clinical Skills and Duties:

  • Active participant in the development of care management standards and care delivery models within the clinic and across OHSU primary care.
  • Responsible for handling urgent/emergent calls and participating in provider visits for assigned patient panel.
  • Perform wound care, INR testing, IV placement/infusion, bladder catheterization, suture removal, injection medication, and other clinical duties as directed.
  • Execute nurse visits focused on, but not limited to: hypertension management, diabetes coaching, wound care, suture removal, anticoagulation monitoring, medication safety, injection medication teaching, and initial prenatal visits.
  • Perform hospital and emergency department follow‑up phone calls to ensure safe transitions of care for patients.
  • Participate in yearly Risk Stratification to identify high‑risk patients appropriate for care management.
