- Graduation from an accredited Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) school of nursing or Physical Therapy Assistant (PTA) program.
- Within 30 days of hire:
National Provider BLS - American Heart Association - Upon hire:
Oregon Licensed Practical Nurse - Upon hire:
Washington Licensed Practical Nurse - if practicing within this state. - 2 years of experience as an LPN/LVN (Acute, Clinic, or Long Term Care).
- 1 year of Home Health experience.
- 1 year of Triage experience.
