Position: LVN / LPN

Job Summary

This position is for a Licensed Vocational Nurse (LVN / LPN) in a home health setting, responsible for providing nursing care under the direction of a Registered Nurse. The role involves gathering and reporting patient data, carrying out orders, and performing therapeutic procedures appropriate for the patient’s age and condition. The LVN / LPN will work in West Portland, OR, and surrounding areas, including Beaverton, with a focus on minimizing drive time between patient visits.

Responsibilities
  • Providing appropriate nursing care as directed by an RN.
  • Gathering and reporting patient data.
  • Carrying out physician orders.
  • Performing therapeutic procedures on patients.
  • Documenting patient care and related duties.
  • Assisting patients with activities of daily living.
  • Collecting specimens.
  • Obtaining and recording vital signs.
  • Communicating patient status and reporting changes in condition to licensed professionals.
  • Maintaining and cleaning equipment.
  • Managing patient care using EPIC Remote Client for Home Health (on laptop) and Rover (phone app) / Oasis.
Required Skills & Certifications
  • Active LVN / LPN license in the state of Oregon.
  • BCLS certification.
  • Minimum of 1 year of skilled home health experience.
  • Valid driver’s license, car insurance, and reliable transportation.
  • Oasis experience.
  • COVID-19 vaccination primary series and one booster.
Preferred Skills & Certifications
  • Experience in emergency or critical care environments.
  • Comfort level working with vents / trachs / PICC lines.
  • 2 years of experience in direct patient care.
Special Considerations
  • Traveler is expected to have their own transportation.
  • Facility can call off up to one shift per pay period (every two weeks).
  • Occasional travel radius of roughly 60 miles (60-90 minute total drive time) per day.
  • Business casual attire, no jeans, scrubs allowed but not required (no specific color).
Scheduling
  • 8-hour day shifts.
  • Shifts are determined and assigned by the facility.
  • No weekend requirement.
  • No on-call requirement.
  • No block scheduling.
