Responsibilities- Plan and coordinate new referrals.
- Provide phone triage for existing patients.
- Deliver direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living sites.
- Collaborate with providers, patients/caregivers, physicians, and payors to deliver services in Oregon and/or southwest Washington.

- Oregon Registered Nurse license (upon hire).
- National Provider BLS – American Heart Association within 30 days of hire.
- License in each state where care is delivered (upon hire).
- Minimum 3 years of home infusion nursing experience.
- Bachelor’s Degree in Nursing.

Salary Range$46.54 – $72.26 (Base pay; additional compensation potential for shift differentials, overtime, and bonuses).
