Description
RN Care Manager (RCM) - Long Term Care, Child Center, at Providence Portland Medical Center in Portland, Oregon. This is a full time, day shift position.
The ideal candidate will have 2 or more years of RN Care Manager work experience in an adult long term care facility.

The Resident Care Manager is accountable for quality of care for residents, according to the individualized needs of the resident, following the nursing process and in accordance with the Nurse Practice Act. Manages the nursing care of assigned residents, ensuring their maximum independence and self-direction. Coordinates care provision with physicians and other Interdisciplinary Team members in a collegial relationship. Ensures that the resident's care plan is developed, implemented and documented to meet their individual care needs.

Required Qualifications:
- Bachelor's Degree in Nursing.

In compliance with the Department of Human Services (DHS), a criminal background check is required for this position (every 2 years). - 3 years Nursing experience, with 2 years in care of medically fragile children and/or adults.
- Recent relevant experience.
Preferred Qualifications:
- Master's Degree in Nursing.
- 2 or more years of RN Care/case management experience in a Long Term Care environment
- Leadership experience.
