Provide nursing care in accordance with physician orders, agency policies, and Oregon regulations. Conduct routine nursing visits and monitor client health status. Perform medication management and medication set-up as permitted by agency policy and state regulations. Provide wound care and other nursing services within LPN scope of practice. Observe, document, and report changes in client condition to the supervising RN. Educate clients and families on health maintenance and safety measures.

Complete accurate and timely documentation.

Current TB Screen and Criminal Record Check. Must be able to legally work in Oregon, USA to apply for this position. Current, unrestricted Oregon LPN license. Home health, long-term care, assisted living, or acute care experience preferred. Strong assessment, communication, and documentation skills. Current CPR certification. Valid driver's license, proof of insurance, and reliable transportation. Ability to work independently and manage a flexible schedule.
