LPN​/RN Clinical Coordinator

A premier regional home health and clinical care provider is seeking licensed nurses (LPN / RN) to join their team as Clinical Coordinators in the Greater Portland, OR metropolitan area. This is a 100% administrative and care-coordination role with no hands-on patient care involved
, making it an ideal transition for nurses seeking a structured, office-based environment. Openings are available across multiple modern clinic offices on both the East and West sides of the Portland metro area.

• Clinical Triage: Perform administrative and telephonic triage for incoming patient visit requests to assess urgency and route care appropriately.
• Administrative & Leadership Support: Assist clinical management with daily operations, staff support, and clinic workflow coordination.
• Lab & Order Management: Receive, organize, and route laboratory results to care teams; track and process delayed or pending care orders to ensure continuity of care.
• Physician & Patient Communication: Act as the primary point of contact for incoming inquiries, query management, and follow-ups from patients, families, and healthcare providers.
• Workflow Efficiency: Utilize Electronic Medical Record (EMR) systems to maintain accurate patient documentation and track care delivery milestones.

• License: Active, unencumbered LPN (Licensed Practical Nurse) license in the State of Oregon required. (RN license holders also considered).
• Experience: Prior experience in Home Health, Hospice, or outpatient care coordination strongly preferred.

  Strong computer proficiency and familiarity with clinical EMR software (experience with Home Care Home Base or similar systems is a plus).
• Attributes: Excellent communication skills, strong attention to detail, and the ability to operate independently in a fast-paced environment.

• Schedule: Monday – Friday, 8:00 AM – 5:00 PM (Full-Time, 40 hours/week)
• Location: On-site office setting (Options available in Portland Metro East & West sub-markets)
• Pay Range: $26.55 – $40.62 per hour (commensurate with experience)
