Case Manager

The Case Manager will work across various units and will be responsible for managing a caseload of approximately 20 clients. This role requires flexibility and adaptability as it involves floating between different units as needed.

• Manage and coordinate care for clients across multiple units.
• Assess client needs and develop appropriate care plans.
• Collaborate with multidisciplinary teams to ensure comprehensive client care.
• Maintain accurate documentation of client progress and outcomes.
• Conduct regular follow-ups to monitor client needs and adapt care plans accordingly.

• Weekends are required every other weekend.
• Block schedules will be implemented as part of the role.
• On-call duty is not a requirement for this position.

• Proven experience in case management or a related field.
• Strong organizational and communication skills.
• Ability to manage multiple priorities and adapt to changes.
• Knowledge of relevant regulations and best practices in case management.

This position does not permit a work schedule of 48 hours.
