As the Social Services Director, you will identify and address each resident’s social, emotional, and psychological needs, facilitate the development of the resident’s full potential, and assist in planning for discharge.
How You Will Make a Difference- Assist residents and families with social, emotional, and family issues.
- Maintain timely and accurate documentation in compliance with Facility, State, and Federal policies.
- Participate in resident care planning.
- Develop discharge plans and prepare residents for transition.
- Serve as a general source of information for residents and families.
- Master’s Degree in Social Work or Psychology.
- 3+ years of experience in Social Services, preferably in a healthcare setting.
- High level of professionalism and ability to maintain confidentiality.
Bachelor's or Master’s in Social Work or Psychology.
