• Active LPN license in TX
• Current BLS certification
• Hospice or palliative care experience preferred
• Strong communication and clinical assessment skills

• Provide compassionate bedside nursing care to hospice patients
• Administer medications and treatments as prescribed
• Monitor patient comfort and symptom management
• Document care in EMR accurately and in a timely manner
• Communicate with the care team and families as needed
• Maintain compliance with hospice standards and HIPAA
• Respond promptly to patient needs and end-of-life concerns
• Perform other duties as assigned
