Key Responsibilities

The Youth & Family Services Outreach Coordinator willدمات to children, adolescents, and their families while supervising a team of Family Outreach Specialists. Family Outreach Coordinators and Family Outreach Specialists provide therapeutic behavioral supports, family support services, and targeted case management to eligible youth and their families. Services are delivered in the home, school, and community settings. Family Outreach Specialists and Coordinators work within a team model which includes clinical senare, case managers, therapists, prescribers, and more.

Family Outreach Specialists and Coordinators collaborate with external supports such as school staff, community supports, DJJS/DCYF staff, and more. Family Outreach Coordinators will provide individual and group supervision, in‑vivo supports, and support the professional development of the Family Outreach Specialists with support from YFS Clinical Managers. The Family Outreach Coordinator will assist in developing and organizing summer programming and school‑based skills groups for patients.

The Family Outreach Coordinator will be an active member of both internal and inter‑agency meetings.

Job Requirements
  • Required minimum education of a bachelor’s degree in social work, psychology, human services, or an equivalent field of study.
  • Experience working with children and families in a mental health or human service setting required.
  • Previous supervisory experience strongly recommended.
  • Reliable transportation needed, as community‑based services include transporting patients.
  • Must be available for weekday evening hours.
  • Effective communication skills to collaborate with internal and external supports and work within a team model.
  • Ability to establish and maintain positive relationships with patients, families, community partners, and YFS team members.
  • Commitment to the compliant provision of high‑quality services following center, state, and federal guidelines.
