** We are currently seeking a dedicated and compassionate
** Registered Nurse
**** Case Manager
** to join our Home Health team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for Home Health patients and their families.

* Qualifications:

* Education:

** Graduate of an accredited nursing program with a current RN license.
* *
* Experience:

** Minimum of two years of RN experience required. Previous home health experience is strongly preferred.
