Then HIOW Healthcare NHS Foundation Trust -- Community Rehabilitation Service (CRS) has an exciting opportunity for you to join our dynamic team. This permanent position is well suited to an individual that is looking to provide excellent nursing care to the Portsmouth Community in a thriving and supportive workplace.
CRS is a multi-disciplinary team made from staff employed by Health and Adult Social Care for residents of the Portsmouth City area. The service aim is to help individuals remain at home and to avoid unnecessary admission to hospital, and to facilitate discharge from hospital by offering assessment of immediate needs.
The team work hard to ensure high quality person-centred care is at the heart of everything we do, and our staff are supported in their roles and encouraged to develop. You will be required to work within the wider multi-disciplinary team providing evaluating and communicating effectively with other teams within the Service.
Main duties of the jobYou will work as a member of a multidisciplinary team alongside service users to promote independence and daily living skills, which will include supporting with personal hygiene needs, diet, fluids and meal preparation, encouraging them to engage in social or leisure activities as identified within their Care Plan
This role will provide support in management of risk, developing therapeutic relationships, supporting people to live meaningful and fulfilling lives.
Act as a role model for team members, supporting new or less experienced colleagues.
This role will assist and support qualified staff and carry out assigned tasks as directed, without supervision
This will include housekeeping duties where required
This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support
