Flexible bank home-visiting role | Daytime hours | Supportive clinical environment
We are seeking experienced Nurses and Paramedics to join our bank home visiting service delivering clinical care to patients in their own homes across Portsmouth. This role offers flexible daytime working within a clinically led, GP-connected community service.
The service is designed to support autonomous practice, with clear referral pathways from GP practices, defined escalation routes, and access to clinical and operational support throughout each shift. Bank clinicians are valued members of the team and are supported to deliver safe, effective care within a well-governed framework.

Main duties of the jobThe post holder will manage a mobile caseload of home visits, working independently across Portsmouth while maintaining close communication with GP practices and community services. The role involves making appropriate clinical decisions, developing management plans, and ensuring timely escalation or follow-up where required.
The post holder will undertake autonomous clinical assessment and prescribing within their professional scope of practice, in line with the Human Medicines Regulations, NICE guidance and local prescribing policies. All prescribing decisions must demonstrate safe, evidence-based practice and be appropriately documented within the clinical record.
The post holder will maintain accurate, contemporaneous clinical records, contribute to effective clinical handovers, and work in accordance with professional standards, regulatory requirements and local governance frameworks. Effective time management, safe travel between visits, and strict adherence to lone working and safeguarding policies are essential to the role.
Bank clinicians will have access to supervision and clinical support and may undertake repeat shifts subject to service requirements.

Job responsibilitiesClinical Duties
- Undertake face-to-face clinical assessments in patients homes following referral from GP practices
- Diagnose,treat, and manage acute and chronic conditions within scope of practice
- Formulate and implement appropriate care plans, including safety-netting and escalation
- Prescribe medications (where qualified) or recommend treatment in line with PGDs andlocal policy
- Identify deterioration and elevate appropriately to GPs, urgent care, or emergency services
- Provide advice, reassurance, and health education to patients and carers
- Support admission avoidance and early discharge pathways
Professional Practice
- Work independently and autonomously,managing a mobile caseload
- Maintain accurate, timely, and legible clinical records on relevant systems
- Practice in accordance with NMC / HCPC standards,local clinical governance, and policies
- Participate in clinical supervision, reflectivepractice, and continued professional development
- Contribute to audit, service improvement, and quality initiatives
Communication& Team working
- Communicate effectively with GP practices,community services, and wider MDTs
- Provide clear verbal and written clinical handovers
- Build positive working relationships with patients,carers, and professionals
Health,Safety & Governance
- Drive between visits across PO1PO6, managing time effectively and safely
- Ensure lone working procedures are followed at alltimes
- Adhere to safeguarding adults and children policies
- Maintain infection prevention and control standards
