Senior Community Mental Health Nurse - UCR

We are looking for an experienced and compassionate Senior Nurse to join our friendly Urgent Community Response (UCR) Team based in Portsmouth.
In this role, you will work alongside patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.
As a Senior Nurse, you will be responsible for working alongside the Hospital at Home (Frailty Virtual Ward) MDT, influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.

Main duties of the job- Provide mental health expertise.
- Assess patients, evaluate programmes of care and carry out treatment plans.
- Evaluate clinical outcomes
- Contribute to effective discharge planning and/or referral processes
- Apply diagnostic reasoning and professional judgement to nursing practice issues/decisions.
- Manage care of patients/clients on a day to day basis
- Accurately document assessments of patient health status, on the Trust Clinical Records System.
- Know how to refer to the local authority adult safeguarding service.
- Ensures effective collaboration and communication with multidisciplinary teams,
- Actively participate in student placements and staff training within.
- Maintains and develops own clinical expertise and knowledge in speciality evidence-based nursing practice and personal portfolio to demonstrate fitness to practice in line with expectations for revalidation.
- Actively participate in clinical and management supervision.

Person Specification Qualifications- BSc Nursing RGN/RMN currently registered with NMC.
- Post registration, specialist training relevant to speciality.
- Practice assessor/supervisor
- Teaching Qualification eg. ENB 998 or equivalent
- Dual trained
- Experienced in assessing and developing patient care plans.
- Experienced in developing and delivering care and clinical interventions to patients.
- Experience of MDT working.
- Experience of teaching, training and supervision/assessing junior staff and students.
Evidence of recent CPD. - Evidence of developing policy, guidelines contributing to management of resources.
- Excellent communication skills (verbal, non-verbal and written skills)
- Demonstrate in-depth knowledge of nursing procedures
