Cancer Care Coordinator

Working under the guidance of the Lead Nurse and Clinical Nurse Specialists, you will help patients navigate the complexities of their treatment and follow-up care, ensuring they move smoothly onto their Risk Stratified Follow Up (SFU) pathway.

You will be instrumental in co-ordinating appointments, assessments and investigations, supporting holistic needs assessments, promoting self-management, and ensuring every patient experience reflects dignity, respect and the highest standards of safety.
