An exciting opportunity has arisen for a Band 6 Neuro Occupational Therapist to work within the Community Neurology Service (CNS), based out of St Mary's Community Health Campus in Portsmouth
We are looking for a motivated and skilled Occupational Therapist who is committed to working as part of a Neuro rehabilitation multi-disciplinary team. This role will be focused on delivering neurological assessment and treatment plans centred around patients' goals to improve independence and quality of life.
The long-term conditions subdivisions of the Community Specialist Division of Hampshire and Isle of Wight Healthcare NHS foundation Trust delivered a wide range of clinical services across Portsmouth and Hampshire providing personalise, integrated care that supports health and wellbeing for those patients in our care.
The Community Neuro Service, based at St Marys Community Health Campus Portsmouth offers neurological assessments and treatment plans centred around patients' goals to improve their independence and quality of life and management of their long-term neurological condition.
Main duties of the job- Provide specialist, evidence based Occupational Therapy assessments an intervention to a caseload of adult patients to ensure good clinical outcomes.
- Independently case management, advanced clinical reasoning, and collaborative work within a multidisciplinary community neuro team, aiming to optimise functional independence, safety, and participation in daily occupation taking into consideration fluctuating and long-term neurological conditions.
- Develop, deliver, and evaluate individualised treatment plans, focusing on meaningful occupations and functional goals and interventions around:
- Cognitive rehabilitation
- Upper limb rehabilitation and functional retraining
- Fatigue and anxiety management
- Seating, postural management, and pressure care
- Environmental adaptation and equipment provision
- Falls prevention and risk management
- Vocational Rehabilitation and accessing community
- Complex manual handling management
- Risk assessments and consideration of safe guarding/mental capacity implications.
- Participate in delegated management tasks for the team such as providing clinical supervision, recruitment and selection, induction of new starters, management of resources etc, ensuring good working practices for staff, efficiency and cost effectiveness. Implement Trust and service policies and proposed changes.
