Guardian Jobs is seeking an experienced, registered Social Worker to join the Queen Alexandra Hospital Social Work Team. You will support adults leaving hospital by coordinating assessments and services with health and social care colleagues to achieve the best outcomes.
You will conduct comprehensive assessments, manage a complex caseload, and provide supervision and guidance on challenging cases. A SWE registration is required;
Newly Qualified Social Workers are not eligible.
