Overview
Location: Naval Medical Center Portsmouth, VA
Hours of Performance: Services shall be provided Monday through Friday, between the hours of 0700 and 1600 hours.

Responsibilities- Assist in defining patient population in the Medical Home Port (MHP) Team requiring professional nursing intervention and use evidenced‑based clinical practice guidelines to evaluate and provide nursing care for selected patients.
- Work directly with selected patients and families to clinically direct, plan, coordinate, implement and evaluate a plan of care to meet needs. Collaborate with other health care disciplines to provide total quality patient care. Communication avenues may include, but are not all inclusive of asynchronous messaging to improve communication, telephone consultations and other nontraditional means.
- Function as care coordinator to assure all aspects of patient care needs are met, to include assessment, intervention and follow‑up in screening of patient/patient problems.
- Initiate and maintain pertinent patient and family medical history on patients, recording observations and stated symptoms for use by the Medical Home Port Team. Observe and record pertinent vital signs, determine and order laboratory procedures on patients that may be required by the provider to diagnose and treat the patient, per command and department policies. Orient new patients and their families to clinic policies as required.
- Take appropriate action such as practitioner referral and evaluation, health‑care advice, initiation of medical protocols or referral based on medical staff protocols.
- Proactively coordinate the screening of their enrolled patients to identify patients needing preventative services and education based on their specific health‑care needs including, but not limited to health and wellness topics, medical conditions and medical procedures. (Examples include mammograms, A1C, etc.)
- Ensure review of emergency treatment records (ETR) and follow up with patient, per command and department policies.
- Track Health Care Effectiveness Data and Information Set (HEDIS) measures, lab/rad results and notify patients per command/department policies. Check patient identification, Health Care‑Provider’s orders, and transfer information as applicable. Arrange for ordered diagnostic and therapeutic services as directed.
- Administer oral parenteral, intravenous (IV) medications, and maintain annual competencies as outlined in departmental policies.
- Actively participate in MHP Team huddles. Proactively assist team’s identification of diagnostic procedures required during schedule scrub. Assist with identification of patient education needs and review plan with team members for coordination of educational needs 5.5.3.2.6.11. Provide technical direction to licensed practical nurses (LPNs), medical assistants and Hospital Corps personnel.
- Administer specific medications at designated times as ordered.
- Observe patients and report pertinent data on condition and medication or treatments.
- Prepare and administer IVs and monitor their proper function.
- Demonstrate proficiency according to operating manuals when using patient‑care electrical equipment.
- Chart nursing observations and ensure record is complete with all procedures and test results recorded. Maintain required clinic records, patient charts, forms, logs, records, registers, and collect required statistics.
- Perform nursing techniques for the comfort and wellbeing of patients.
- Prepare equipment and assist health‑care providers with treatment procedures.
- Recognize emergencies and respond with appropriate protocol, e.g. resuscitation, oxygen, emergency drugs, cardiac massage, application of medical equipment, as established by the medical staff.
