Northwest Specialty is seeking a compassionate, patient-focused Patient Navigator (LPN) to join the team!
Under the supervision of the assigned Leader, the Patient Navigator partners and collaborates with interdisciplinary team members to coordinate patient access, care progression, education, and communication for patients within the assigned population to ensure continuity of care with compassion, commitment, and connection.

ensures the integrity and confidentiality of information pertaining to patients, staff, and providers is protected. The Patient Navigator projects a professional demeanor and appearance while maintaining confidentiality of patients, coworkers, and the organization as appropriate. This position requires strong critical thinking skills, excellent customer service, and the ability to prioritize in a fast-paced healthcare environment. Works under stress and in situations that demand patience, empathy, and tact while providing exceptional service.
Essential Functions:
• Serve as a primary point of contact for patients and families to facilitate timely access to care and services.
• Provide guidance, education, and support to patients throughout their care journey, addressing barriers to care such as scheduling, transportation, and resource access.
• Utilize telephonic and electronic communication to triage patient concerns and coordinate appropriate follow-up.
• Ensure continuity of care while prioritizing patient safety, experience, and satisfaction.
• Communicate with referring provider offices, internal departments, and external partners as required.
• Tracking referrals, authorizations and patient progression through the care continuum, escalated as needed.
• Identify opportunities for process improvement and collaborate with leadership to enhance the patient and family experience.
• Evaluate with the care team the patient coordination and processes to identify opportunities for improvement in all aspects of the patient and family experience.
• Collaborate with inpatient and other nursing staff in conjunction with Professional Practice Specialists to provide continuing education for the patient and family.
• Demonstrate skill in navigating and communicating within a complex healthcare environment.
• Other duties as assigned.
Qualifications and
Preferred Experience:
• Demonstrates eligibility for employment in the United States.
• Proficient computer skills including MS Office Suite (Excel, Word and Outlook).
• Excellent interpersonal, verbal, and written communication skills.
• Demonstrates strong organizational skills along with attention to detail. Must be highly organized and able to focus on the task at hand.
• Possesses strong analytical and critical thinking skills; excellent data analysis skills and/or the ability to read and interpret data, principles of validity and reliability.
• Current LPN licensure in the State of Idaho is required.
• Proof of Basic Life Support (BLS) Certification.
• Minimum of 2 years’ LPN experience.
• Previous experience in a patient navigator, care coordination, medical office, or healthcare support role.
• Experience working with referrals, authorizations, scheduling, or patient advocacy.
